Home modification and assistive technology (AT) assessments look at how a person's home environment and equipment either support or limit their independence, safety, and participation in daily life. GRS occupational therapists conduct these assessments on-site wherever possible, since seeing the actual home, bathroom, entryways, and daily routines in context leads to far more accurate and practical recommendations than an assessment done remotely.
What to Expect
An assessment typically involves a home visit where our occupational therapist observes the person moving through their usual daily tasks — showering, toileting, getting in and out of bed, entering and exiting the home — and identifies specific barriers to safety and independence. From there, we prepare a report recommending appropriate equipment (such as shower chairs, rails, or hospital beds) and, where needed, home modifications (from minor changes like grab rails through to complex modifications like ramps or bathroom redesigns), scaled to what's actually necessary and proportionate to the person's needs and NDIS plan.

Yes — we assess and report on both minor modifications (like grab rails or lever taps) and major/complex modifications (like bathroom redesigns, ramps, or doorway widening), scaled to what the participant genuinely needs.
Does the assessment happen at my home?
Wherever possible, yes. An on-site visit lets our occupational therapist see the actual environment and equipment in use, which leads to more accurate and practical recommendations than an assessment conducted off-site.
How does this connect to my NDIS plan?
Our reports are structured to align with NDIS requirements for home modification and assistive technology funding, so support coordinators, plan managers, and the NDIA can review and action recommendations directly.
What happens after the assessment?
You'll receive a written report with prioritised recommendations. Our team can also help clarify next steps, including how to source recommended equipment or engage a builder for approved modifications.
